Coordinated Family Supports

Coordination and support for adults with developmental disabilities who live in their family home, and for the families who support them.

About the service

What Coordinated Family Supports are

Coordinated Family Supports are designed for adults with developmental disabilities who live in their family home. The service brings coordination and support to the adult and the family together.

An HSN coordinator works one-to-one with each family to strengthen the natural supports already in place, find and use generic resources (the community, educational, and healthcare services open to everyone), and build stability for the long term.

Families know their loved one best. We plan alongside them, honor their language and culture, and keep the adult's own goals at the center.

Five focus areas

How The HSN supports adults and families

Every family is different. Support is shaped around the adult's goals and the family's daily life, in five core areas.

  • Community resources

Navigating and coordinating generic community, educational, and healthcare resources

  • Household routines

Establishing structured household routines to reduce caregiver stress

  • Planning ahead

Proactive future planning, life-stage transitions, and emergency preparedness

  • Connection and belonging

Fostering natural community integration and social connection

  • Working as one team

Close collaboration with Regional Center Service Coordinators and family circles of support

Eligibility

Who is eligible

This service is a fit when each of these is true:

  • Adults 18 and older
  • Living in the family home
  • Served by a California Regional Center
  • Service authorized by the Regional Center

Coordinated Family Supports work alongside, and do not replace, Regional Center service coordination.

Six steps

How to get started

Every family's path starts with their Regional Center service coordinator. From there, it usually looks like this:

  1. Talk with the service coordinator

The adult or family asks their Regional Center service coordinator about Coordinated Family Supports.

  2. Referral to The HSN

The service coordinator sends a referral to The HSN.

  3. Regional Center authorization

The Regional Center authorizes the service in the IPP. Hours are set through the IPP authorization and family tier assessment.

  4. Getting to know the family

An HSN coordinator meets with the adult and the family to learn about routines, strengths, and priorities.

  5. A shared support plan

Together, we write a plan that follows the IPP and the adult's own goals.

  6. Ongoing support and check-ins

Regular check-ins keep the plan current, in close contact with the Regional Center service coordinator.
