About The Heritage Solution Network

Person-centered care that honors cultural heritage, personal choices, and individual goals.

Our Mission

The Heritage Solution Network LLC provides person-centered services—including Coordinated Family Supports (CFS), Independent Living Services (ILS), and Homemaker Services—that empower adults with developmental disabilities to live safely, successfully, and with dignity. Our mission is to strengthen each individual’s natural supports, cultivate autonomy, reduce barriers to independence, and promote active, meaningful participation across home, community, cultural, and social environments.

We deliver flexible, individualized, and culturally responsive services that honor the unique heritage, language, values, and personal choices of each individual and their family. Anchored in a steadfast person-centered philosophy, we ensure every service design, action, and support strategy directly aligns with the individual's self-identified priorities, goals, and strengths.

Our comprehensive service continuum guides all aspects of our work. The Heritage Solution Network LLC collaborates closely with individuals, their circles of support, Regional Centers, and generic resource agencies to establish holistic, coordinated plans. We operate with unwavering integrity, accountability, cultural humility, and transparency. Our staff undergo comprehensive, ongoing training and supervision to ensure consistent adherence to person-centered thinking, Title 17 standards, and Department of Developmental Services (DDS) requirements.

Through purposeful collaboration, compassionate guidance, and responsive service delivery, The Heritage Solution Network LLC ensures that every individual and family we support has the tools, stability, and confidence to thrive on their own terms.

Our Approach

At The Heritage Solution Network (The HSN), we believe meaningful support begins with listening. We take time to understand the unique aspirations of each individual, the values of the family, and the collaborative vision of the Regional Center service coordinator.

Because independence and family stability look different for everyone, our approach is holistic, responsive, and grounded in dignity. Whether coordinating complex family systems, teaching functional independent living skills, or maintaining a safe, organized household, we build strategies around personal choice and real-world outcomes.

Core Pillars of How We Serve

  1. 01

Person-Centered Planning Aligned with the IPP

Every service plan begins with the individual's self-identified priorities and directly aligns with the goals outlined in their Individual Program Plan (IPP).

  2. 02

    Culturally & Linguistically Responsive Support

We honor each family’s cultural heritage, communication preferences, and home traditions, ensuring care is respectful, familiar, and delivered with cultural humility.

  3. 03

    Proactive Regional Center Collaboration

We maintain close, transparent communication with Service Coordinators, Circles of Support, and generic resource agencies to ensure continuous alignment and seamless advocacy.

  4. 04

    Strengths-Based & Family-Centered Focus

Rather than focusing on limitations, we identify and build upon existing capabilities, reinforcing natural support networks and easing caregiver strain.

  5. 05

    Individualized Skill-Building & Coaching

From home management and financial literacy to community mobility and self-advocacy, our coaching is hands-on, practical, and paced to each person's learning style.

  6. 06

Continuous Monitoring & Responsive Plan Adjustment

Needs evolve over time. We conduct regular progress reviews, adapt strategies as milestones are met, and pivot proactively to address new challenges.
